Burnham AL53A leak

I have a Burnham AL53A that is bubbling water from the bottom of the tank. Nothing at the connections that I can see. I pulled some of blue plastic back to expose the Styrofoam and it appears to be oozing from somewhere within. Curious if this could be a tank failure or the pressure relief valve which is at the top of the tank causing water to work its way down. Any experience with this or a possible trouble shooting technique? Any problems common to this model? To be fair, it was installed in 2001 and may just need replaced. Thanks

I have diagnosed it to be a corroded hand hole plate/cover. Any one know where I can get a replacement. Internets searches have been negative. Thanks.

Its a Burnham AL53A INDIRECT WATER HEATER
