Door to Underground Shelter

I have a 12'x30' underground storm shelter under my back deck. I have not poured the concrete cap yet and need some ideas on a hinge for the door. My plan was to have a 2" lip recessed in the concrete for sealing with a steel door that will be flush with pad when closed. I do not want my hinges above the door to trip on every time I am on the deck. Anyone have any ideas on how to make some hinges for a flush mount door? Any ideas, complex or simple are welcome. The more I work on this the more complicated the hinges become. I need some fresh ideas.

Probably too late for this idea, but why not have the door for the shelter a little lower beneath the deck, and have a hinged or removable section of decking over it. Then you could use simple but beefy hinges on the shelter door with no worry of tripping, and the whole thing would be out of site yet easy to get to.

Any Marine supply will have recessed hinges and and pulls.
West Marine, Port Supply, James Town Distributors, Overtons to name a few.

Although we can't see your project, would not a Bilco door suffice, or do you not have the space for it?

I would look at a Bilco steel floor access door.
