Reuse of Tapcon holes?
I know you can't back out a Tapcon screw and drive it back in again because you can't reuse the hole.
However, I have also seen quite frequently people stick a tooth pick or a strip of wood into a "previously used hole" and drive the Tapcon in again.
Is this really kosher?

No... a toothpick is not going to be of much use in masonry.
If you can't move the hole you can switch to the next larger diameter Tapcon.
If you can't move the hole you can switch to the next larger diameter Tapcon.

Ive used some small gauge aluminum wire inserted into a slightly oversize hole and it held very well.

You can reuse the hole sometimes, but it depends on the psi of the concrete, how much screw is in the concrete, and what is being fastened. Obviously if the screw spins you need another.
Avoid the 3/16 tapcons... I only like to use the 1/4". A tapcon is really just a type of wedge anchor.
