two handle faucets tub/shower
Are two handle faucets "universal" sized? Is finding a replacement
valve that will fit the cutouts in the tile usually an issue? (replacing from back).

Most name brands are the same spacing, BUT you can always install a single handle valve and use a cover plate to occlude the two holes on the side. Will require a little tile cutting, but doable. Renovation Cover Plate in Chrome-RP29827 - The Home Depot

Check out the close proximity of the spout to the handles. Does that seem "unstandard"? Would retrofitting a single faucet be an issue because of this close proximity?

8" center is normal. Older ones can be as small as 3 3/8" center. Can you measure yours? If 8" you could replace what you have, but I do like the newer look of a single control valve.

So if its 8" then I am ok with a newer 2 handle but anything less I am going to have trouble so I should consider a single?

Yeah, I would measure center to center of the stems and see what is out there that would suit your fancy with those same measurements. Single control valve will require cutting some of the tile, but would look so much better.
