Vinyl look flooring over hardwood.

I have a house in Louisiana that is about a 100 yrs old. It’s elevated about 3 ft off the ground. No subfloor. Sections of the hardwood have to be replaced due to them being weak or have holes. Going to redo the weak/missing sections with same thickness plywood and then have the old hardwood floors become my subfloor. What should be my next step? I’ve heard to ad some sort of moisture barrier then put laun or plywood sheets on top then add some sort of thin cushion/under layment then the vinyl planking. Any advice please? These are the vinyl floor planks I bought.

So did you read the installation instructions to see what the manufacturer recommends?
